What supportive documentation is required to apply for an assessment accommodation?


Below is a summary of the support documents required:

Assessment Accommodations/special educational needs

  • Letter from school regulatory body (IEB, GED, WCED).
  • Letter from a recognised assessment body.
  • Letter from the school confirming accommodations granted.

Reports from Educational Psychologist (not older than 12 months) which must provide details of the standard assessments used as per PDIIE026 Annexure A (The IIE Policy Bank). Please reach out to your Student Relations Navigator if you are not able to download Annexure A. 

Accommodations for severe physical condition

  • A report from a relevant healthcare practitioner.
  • There must be a relationship between the physical impairment and specialist physician.
  • Report must include the diagnosis, treatment, management, support and justification of special concession/s.
  • Evidence indication hospitalisation

Accommodations for severe mental health condition

  • A report from a psychologist and/or psychiatrist.
  • Report must include the diagnosis, treatment, management, support and justification of special concession/s.
  • Evidence of multiple hospitalisations.

Accommodations for short-term physical or medical difficulties or injuries

  • Letter from a relevant healthcare practitioner stating the nature of the injury. 
  • Evidence of hospitalisation. 
  • Medical reports such as x-rays.
