To complete the application, please ensure you attach all the necessary supporting documents: 1. Accommodations for invigilated assessments such as test/exams are based on what was granted in the school leaving examinations:
- School regulatory body (GED, WCED, IEB, etc.) letter stating which accommodations were granted for school-leaving examinations must be provided.
- Recent (not older than 12 months) educational psychologist report or official letter from a recognised assessment body confirming approval of concessions
2. Accommodation for students who have a serious chronic, physical and/or mental condition that is ongoing, having lasted more than one year that impacts daily living and directly impacts assessment performance: - A recent report from a relevant health care practitioner that fully addresses the following (Please note: General Practitioners (GPs) are not considered to be specialist in managing chronic conditions):
- The diagnosis – when the diagnosis occurred and expected duration of the condition
- Specify the treatment involved in the condition
- Management and support of the condition, with details about how the student is managing the condition and any support mechanisms that are in place.
- Justification of the special concession/s that provide sound reasons, based on the condition, for why and how the condition impacts assessment performance.
